What is Find Care?

Find Care is our online search tool that helps you find the right AHN health care professional for your patient. Search by name, specialty, or expertise; find convenient practice locations and read reviews. Click the name to select the right health care professional for your patient, click the Refer Your Patient tab on their profile, and complete their online referral form.

How do I find all locations for a particular specialist?

The specialist’s primary practice location is displayed in their search listing. From there you can click on the More Locations drop down to see a list of all practice locations with phone and fax numbers.

You can also click on the specialists’ name in the search results to open their profile then click the Locations tab to see a map with all practice locations marked. Click on each marker to see the full location address and phone number.

What if I can’t find the right specialist in Find Care or need additional assistance?

1. If you can’t find the AHN specialist you’re looking for:
2. Make sure you spelled their last name correctly and try again.
3. Try a search by their specialty or expertise and review the search results.
4. For additional assistance, or to verify the specialist’s availability and complete a referral by phone, call 1 (844) MD-REFER (1-844-637-3337), Monday – Friday from 7:00 AM to 5:00 PM.

IMPORTANT NOTE: There are two referral types you should not make using Find Care or the MD-REFER phone number. Click the relevant link below for the specific referral steps for each service.

1. Refer a patient for Electroconvulsive Therapy (ECT).
2. Assist a cancer patient with a Second Opinion Request.

What happens after I complete a referral for my patient? How do I follow up on their test results, diagnosis, or care and treatment plan?

Use your EpicCare® Link™ account to keep track of your patients’ care after completing their referral to an AHN specialist. Your EpicCare Link account allows you to:

• Access your patient’s full AHN medical record.
• Send and receive messages directly with specialists.
• Get real-time notifications and updates on your patients, including test results, hospital admissions, discharge summaries, and more.

Credentialing Information

If you would like to apply for appointment at privileges at an AHN Hospital or Surgery Center, complete a credentialing request via our Provider Intake Form.

Hospital Affiliation Verification

To obtain a hospital affiliation verification for an AHN Facility, please email AHNCVS@ahn.org, with the following:

• Last name
• Email address
• Phone number
• Organization
