Ambulatory Pharmacy Specialists

The embedded clinical pharmacists engage in Chronic Medication Management to optimize medication regimens and improve clinical outcomes. Disease states include diabetes, hypertension, hepatitis C, weight management and more. Pharmacists also counsel patients on medications and/or devices, assisting with quality metrics, educating clinical staff, and answering drug information questions. Pharmacists also assist with practice protocols and policy development to improve clinic workflows and reduce the risk of medication related errors.
