Support services for patients and their loved ones

A cancer diagnosis is difficult enough. You deserve the opportunity to heal — without the stress of chasing down the care and resources you need.

That’s why AHN’s support services start with navigation. Our unique Cancer Navigation Team handles all the details for you, coordinating everything you might need at hospital and home, including all the services described below.

AHN Cancer Navigation Team

Your entire cancer care experience will be coordinated and managed by the AHN Cancer Navigation Team, from diagnosis to treatment, to survivorship and recovery. 

AHN knows that cancer impacts all areas of your life. The Cancer Navigation Team manages all the details — getting you set up with the care and support you need so you can focus on healing.

Your Cancer Navigation Team includes:

  • Your primary nurse navigator: An oncology nurse specializing in the care and services you’ll need throughout your unique journey. They act as a single point of contact throughout your care.
  • Financial counselors: AHN experts who work to minimize financial stress. They answer billing and insurance questions, coordinate payment plans, and connect you with resources for financial assistance.
  • Social workers: A dedicated team offering mental health counseling, connection to support groups, and legal planning.
  • Community navigators: Helping you and your family with transportation and travel logistics.
  • Dietitians: Providing counseling and advice to help manage symptoms and side effects, and to ensure patients get the nutrients they need specific to their conditions.

Genetic testing and counseling

Most cancer is spontaneous without any link to family history or genes. But in some cases, cancer is caused by an abnormal gene passed along from a parent.

For those who may be affected in this way, we offer specialized expertise in genetic testing and counseling. Our genetic counselors conduct in-depth interviews with you and provide options for preventive care, including: 

  • Lifestyle changes.
  • More frequent screenings.
  • Medications to reduce the likelihood of developing cancer.
  • Surgery to remove at-risk organs, such as breasts, ovaries, or the colon.

If you have any of the following, you might consider genetic testing for yourself:

  • Cancer of any type in more than one of your immediate relatives (mother, father, sister, brother)
  • Cancer in your family that is known to be linked to a single gene mutation (for instance, breast, ovarian, and pancreatic cancer)
  • Family members who developed cancer at a young age
  • Close relatives with rare cancers that are linked to hereditary conditions
  • A physical finding that is linked to hereditary cancer (such as an abnormal amount of colon polyps)
  • A known genetic mutation in the family found through testing

If you are not currently a cancer patient but are concerned you could be at risk, talk with your family members. Ask who in your family has had cancer, where the cancer began in the body, and what age your relative was when diagnosed. Then, complete a family history questionnaire

Pain management and palliative care

AHN offers a multidisciplinary palliative care team that addresses the physical, emotional, and spiritual needs of cancer patients and their families. 

Unlike hospice — which is end-of-life care — palliative care aims to improve your quality of life as you battle the disease. We treat symptoms of cancer and the side effects from treatment, such as pain and nausea, and help you to cope mentally with the disease.

Our team can help you with:

  • Making treatment choices and other care decisions.
  • Emotional and spiritual support.
  • Pain and symptom management.
  • Planning for post-hospital care at home or in an inpatient facility.
  • Patient and family counseling.

We provide palliative care on both an inpatient and outpatient basis and can help families coordinate various support services for their loved ones.

Oncology rehabilitation

Oncology rehabilitation helps cancer patients maintain their function and enhance their quality of life throughout treatment and recovery. Our specially trained AHN therapists collaborate closely with your physicians and keep them fully informed of your progress.

Cancer registry

Cancer registries are large databases maintained by accredited cancer programs like the AHN Cancer Institute to help oncologists determine the best way to treat each type of cancer.

Our tumor registry collects demographic, diagnostic, treatment, and outcome data that is thoroughly analyzed by AHN providers and administrators, as well as outside researchers. The registry electronically transmits cancer data to the Pennsylvania Cancer Registry and to the National Cancer Data Base to advance cancer care across the U.S. and around the globe.

AHN’s tumor registry is accredited by the American College of
Surgeons/Commission on Cancer.

The Patient Ambassador Program

The Patient Ambassador Program connects you with former Cancer Institute patients who volunteer their time to help current patients living with cancer. They’ll provide peer-to-peer support throughout your treatment, along with advice, fellowship, and friendship from someone who has been there.

The AHN Cancer Survivorship Program offers one-on-one support, focusing on your ongoing physical health and emotional well-being as you face the uncertainties of life after cancer.

Hospice care

When a cure for our patients is no longer possible, AHN provides hospice care for those nearing the end of life.

Our team of nurturing, compassionate professionals gives comfort to terminally ill patients by minimizing their pain and creating a peaceful environment where they can live their final days. The hospice team also works closely with each patient’s family to offer caregiving support and emotional encouragement as they deal with the stress and sadness of losing their loved one.
